Abstract:

Introduction:  1. Need for the Activity
With population aging and the rise in chronic diseases, home care has become increasingly complex and specialized, necessitating the development of a structured practice-based education program to systematically strengthen the clinical and educator competencies of home advanced practice nurses (APNs).

2.Problem Analysis and Objectives
This project aimed to address the lack of practice education experts and standardized educational materials for managing diverse home-care patients by establishing specific practice education topics and a structured training system to enhance APNs’ educational competence and satisfaction.

Methods: Improvement Activities

1.Selection of practice education topics for home advanced practice nurses
Drainage Tube Care, Wound Care, Elimination Care, End-of-Life Care, Nutritional Care, Respiratory Care

2.Selection of experts according to the practice education content
One or more experts were selected according to the specific clinical practice education content

3.Determination of educational methods and content by topic, along with the development of practical training modules
To enhance learning comprehension, Presentation incorporating visual aids and instructional videos demonstrating homebased procedures were utilized

4.Implementation of mock lectures and simulation-based practice by practice education experts
Following the lecture and Q&A with feedback, practical training was conducted

5.Feedback through internal satisfaction surveys
Satisfaction was assessed regarding program necessity and structure, educational content and methods, practical application and confidence improvement, and additional suggestions

Results: This improvement initiative enhanced the clinical performance and self-confidence of home advanced practice nurses by establishing a structured and standardized practice education program, enabling them to independently provide care in home settings and strengthening their roles as clinical educators.

The application of professional practice education is expected to contribute to improved health outcomes and patient safety by delivering skilled and consistent nursing care and education to home-based patients with severe conditions.

Furthermore, a foundation was established for the development and utilization of six specialized practice education programs, including Drainage Tube Care, Wound Care, Elimination Care, End-of-Life Care, Nutritional Care, Respiratory Care.

Conclusion: This initiative established a foundation for professional and systematic practice by developing a practice education expert training program for home advanced practice nurses in response to the increasingly complex demands of home healthcare.
The completed program is expected to enhance the clinical competencies of home advanced practice nurses and to contribute to improved safety and health outcomes for home-based patients with severe conditions through the provision of professional and standardized nursing care.

Biography:

Nurse EunHee Jang is a home advanced practice nurses who visits patients from the hospital to their homes.
She received her Master's degree from the Graduate School of Clinical Nursing at Sungkyunkwan University.
She is currently working as a home advanced practice nurses in the Home Nursing Team at Seoul National University Hospital.
