Interprofessional Care

Interprofessional Care focuses on coordinated collaboration among nurses, physicians, pharmacists, therapists, social workers, and other allied-health professionals to optimise patient outcomes, safety, and satisfaction. This session explores how well-integrated teams reduce fragmentation, enhance communication, and improve continuity of care across clinical settings. Participants at the Healthcare Conference will examine models that support teamwork in acute care, chronic-disease management, rehabilitation, emergency response, and preventive health. Concepts from collaborative care practice reinforce the value of shared decision-making, respect for discipline-specific expertise, and collective accountability.

The session begins by examining how diverse professionals contribute to comprehensive care planning.
